Smartsoft has been working with SIRA over the last few weeks to develop the latest version (August 2016) of the Allied Health Recovery Request form in a Front Desk Document format.

We have now been given approval to distribute it to our users and have made it available to access via the following link: www.smartsoft.com.au/Downloads/AHRR_Form.zip

Once downloaded, you will need to extract the Front Desk Document (FDD) file and add it into Front Desk as a Standard Letter Template. For your convenience, instructions have been included below:

Select System, Standard Letters

Select Add to find the file on your computer.

Select the ellipsis button

, find the extracted AHRR_Form.FDD file and select Open. The file will be downloaded and extracted to your Downloads folder by default

Customise the Description if required, and then select OK
​

The Allied Health Recovery Request form is all ready to be used.
